You need to create items to model shipping charge and insurance charge. These items are of the type "other charge". (Note that even if you are not using Amazon or insurance, you are still required to create these items.)

You also need to create item(s) to model PayPal and Amazon Fees. Since these fees are taken directly out of the sales total, these items are of the type "discount".
